Substitute State Forms

Yearli produces a state-acceptable substitute form for the following states:

  • Arizona
  • Arkansas
  • Delaware
  • Idaho
  • Kentucky
  • Mississippi
  • Missouri
  • South Carolina

Yearli produces a Record Copy Only substitute for the following states:

  • Alabama
  • New Hampshire
  • New Mexico
  • North Dakota
  • West Virginia
  • Wyoming

The substitute form for these states should not be filed with the state agency.

The Substitute State Form contains the Payer's name, address, city, state, zip code, and employer's EIN. The form also contains the Quarter ending date and a list of Employees' TINs, Names, and State Wages.

Creating the Substitute Form

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Click on the Payer State Info button.
  4. Click Add.
  5. Choose the applicable state from the State drop down menu.
  6. Enter applicable state information.
  7. Click Save.
  8. Click OK.
  9. Click on the Add Form button.
  10. Choose the applicable state from the Federal/State/Local drop down menu.
  11. Choose Substitute from the Form Name menu.
  12. Choose the appropriate quarter.
  13. Click OK.
  14. Enter the applicable information.
