Creating the NC-5Q form
- Click on the Payer List button.
- Choose the payer.
- Click on the Payer State Info button.
- Click Add.
- Choose NC from the State drop down menu.
- Enter applicable state information. The State ID for W-2 will be the North Carolina Account ID.
- Click Save.
- Click OK.
- Click on the Add Form button.
- Choose North Carolina from the Federal/State/Local drop down menu.
- Choose NC-5 from the Form Name menu.
- Choose the appropriate quarter.
- Click OK.
- Enter the applicable information.
Note: When the NC-5Q Prep Screen is accessed for a selected period and a form is not saved for that period, the Page 2 Prep Screen displays prior to page 1.
