How do I create a North Carolina NC-5Q form?

Creating the NC-5Q form

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Click on the Payer State Info button.
  4. Click Add.
  5. Choose NC from the State drop down menu.
  6. Enter applicable state information. The State ID for W-2 will be the North Carolina Account ID.
  7. Click Save.
  8. Click OK.
  9. Click on the Add Form button.
  10. Choose North Carolina from the Federal/State/Local drop down menu.
  11. Choose NC-5 from the Form Name menu.
  12. Choose the appropriate quarter.
  13. Click OK.
  14. Enter the applicable information.

Note: When the NC-5Q Prep Screen is accessed for a selected period and a form is not saved for that period, the Page 2 Prep Screen displays prior to page 1.
