How do I create the Connecticut UC-5A/UC2 form/

Creating the UC-5A/UC2 form

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Click on the Payer State Info button.
  4. Click Add.
  5. Choose CT from the State drop down menu.
  6. Enter applicable state information.
    • State ID for SUI: The State ID for SUI is the Connecticut Registration Number*.
    • SUI Wage Max: The SUI Wage Max is used to calculate the Excess Wages on Line 2 of the UC-5A/UC2.
    • SUI %: The SUI % is the Contribution rate on line 4 of the UC-5A/UC2. This is used to calculate line 5.
  7. Click Save.
  8. Click OK.
  9. Click on the Add Form button.
  10. Choose Connecticut from the Federal/State/Local drop down menu.
  11. Choose UC-5A/UC2 from the Form Name menu.
  12. Choose the appropriate quarter.
  13. Click OK.
  14. Enter the applicable information. 

*The format of the Connecticut Registration Number is 1234567-123, 12345678-123, 123456789-123, or 1234567890-123. The last three digits should be 000 if you haven't been assigned a 3-digit number.
