How do I create the Colorado UITR-1/UITR-1a form?

Creating the UITR-1/UITR-1a form

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Verify that there is no punctuation in the payer's name.*
  4. Click on the Payer State Info button.
  5. Click Add.
  6. Choose CO from the State drop down menu.
  7. Enter applicable state information.
    • State ID for SUI: The State ID for SUI is the Colorado Employer Account Number.
    • SUI Wage Max: The SUI Wage Max is used to calculate the Excess Wages on Line 10 of the UITR form.
    • SUI %: The SUI % is the Combined Rate on Line 12 of the UITR form.
  8. Click Save.
  9. Click OK.
  10. Click on the Add Form button.
  11. Choose Colorado from the Federal/State/Local drop down menu.
  12. Choose UITR-1/UITR-1a from the Form Name menu.
  13. Choose the appropriate quarter.
  14. Click OK.
  15. Enter the applicable information.
