Creating the 33H/33HA form
- Click on the Payer List button.
- Choose the payer.
- Click on the Payer State Info button.
- Click Add.
- Choose UT from the State drop down menu.
- Enter applicable state information.
- State ID for SUI: The State ID for SUI is the Utah Registration Number.
- SUI Wage Max: The SUI Wage Max is used when calculating the Excess Wages.
- SUI %: The SUI % is the Contribution Rate.
- Click Save.
- Click OK.
- Click on the Add Form button.
- Choose Utah from the Federal/State/Local drop down menu.
- Choose 33H/33HA from the Form Name menu.
- Choose the appropriate quarter.
- Click OK.
- Enter the applicable information.
