How do I create a Michigan UIA 1028 form?

Creating Michigan UIA 1028 form

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Click on the Payer State Info button.
  4. Click Add.
  5. Choose MI from the State drop down menu.
  6. Enter applicable state information. 
    • The State ID for SUI:  UIA Employer Account Number.  Enter the seven-digit Account Number and three-digit Multi-Unit Number in the State ID for SUI field.
      • If the Account Number begins with zeros, include the leading zeros in the State ID for SUI field.
      • If you do not have a Multi-Unit Number, enter “000” for the last three digits.
    • The SUI %: The SUI % is the employer's UI Tax Rate on the UIA 1028 prep screen, and includes the Account Building Component, the Chargeable Benefits Component, and the Nonchargeable Benefits Component (ABC + CBC + NBC).
    • The SUI Wage Max: The SUI Wage Max is the Taxable Wage Limit used in calculating Excess Wages.
  7. Click Save.
  8. Click OK.
  9. Click on the Add Form button.
  10. Choose Michigan from the Federal/State/Local drop down menu.
  11. Choose UIA 1028 from the Form Name drop down menu.
  12. Choose the appropriate quarter.
  13. Click OK.
  14. Fill in the necessary fields on the form.
    • “Apportion payments” checkbox: If there were 25 or fewer employees on January 12th of the prior year and 50% or more of the total previous year’s contributions were payable with the first quarter report, you can elect to distribute your tax due for first quarter in four equal payments (25% due with each quarterly report). 
    • Obligation Assessment Rate: Located on the UIA 1028 prep screen. Enter the OA rate separately from the Contribution Rate. Include other rate factors in the Obligation Assessment Rate field, if applicable (including Non-reporting Penalty Rate, SUTA Penalty Rate, or Solvency Rate).
  15. Click Save.
