How do I create a Georgia DOL-4N form?

The Georgia DOL-4N form is used to report state income tax withheld to the state of Georgia on a quarterly basis.

Creating the DOL-4N form

Note: Abbreviations used in the address areas must meet the U.S. Postal Service's guidelines for abbreviations. Failure to comply with this request may result in a processing delay.

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Click on the Payer State Info button.
  4. Click Add.
  5. Choose GA from the State drop down menu.
  6. Enter applicable state information.
    • State ID for SUI: The State ID for SUI is the Georgia DOL Account Number.
    • SUI Wage Max: The SUI Wage Max is used to calculate the Non-Taxable Wages on line 3.
    • SUI %: The SUI % is the Contribution Tax Rate.
  7. Click Save.
  8. Click OK.
  9. Click on the Add Form button.
  10. Choose Georgia from the Federal/State/Local drop down menu.
  11. Choose DOL-4N from the Form Name menu.
  12. Choose the appropriate quarter.
  13. Click OK.
  14. Enter the applicable information.
