How do I create the Colorado DR 1093 form?

Creating the DR 1093 form

  1. Click on the Payer List button.
  2. Choose the payer.
  3. Verify that there is no punctuation in the payer's name.*
  4. Click on the Payer State Info button.
  5. Click Add.
  6. Choose CO from the State drop down menu.
  7. Enter applicable state information. The State ID for W-2 should be the 8-digit Account Number for the payer, assigned by Colorado.
  8. Click Save.
  9. Click OK.
  10. Click on the Add Form button.
  11. Choose Colorado from the Federal/State/Local drop down menu.
  12. Choose DR 1093 from the Form Name menu.
  13. Click OK.
  14. Enter the applicable information.
